Detailed below are the Medical Home Teams available to our patients.  A Medical Home is not a building, house, or hospital, rather an approach to providing coordinated primary care that is accessible, comprehensive, and personalized.  Our goal with this team approach is for you to have a designated primary care provider and see this provider for every visit.  For those times it is not possible, you may be seen by another provider within your Medical Home Team.  This will help you to become more familiar with your Medical Home Team and your team to become more familiar with you and your healthcare needs.

What are Extended Hours at the Manchester clinic?

Extended Hours are designated to diagnose and treat common illnesses like strep throat, ear infection, urinary tract infection, or minor injuries after normal business hours.

Extended Hours are staffed by ARNP’s or PA’s, offering the same quality care you expect from your primary care provider. If follow up care is required, your appointment will be scheduled with your Medical Home Team to ensure continuity of care is provided.

The Manchester Extended Hours are made available for all patients of our Regional Family Health satellite clinics. Patients should call the Manchester clinic during any office hours to schedule their same-day appointment for Monday-Thursday 5-7pm and Saturday 8am-12pm.

When should you go to the Emergency Room instead?

Anyone having chest pain or uncontrollable bleeding should call 911 or go to the Emergency Room immediately.
